Amanda Warner
RN, MSN, FNP-BC Primary Care Provider
As the primary provider at Perigon Medical, I have over Ten years of experience as a master's prepared Registered Nurse, with a heavy background in the operating room and peri-operative services. I decided to go back to school to become a Provider, and was board certified as a Family Nurse Practitioner in 2019. I have six years of experience in Family Medicine, working in Primary Care and Urgent Care settings, and a passion for mental health and working with patients with a history of substance use, and eating disorders, but love working with individuals of all ages and backgrounds. I look forward to working together to achieve your wellness goals and increasing your quality of life.
My approach is rooted in open communication, building trust, and meeting people where they are in life. I believe that everyone deserves access to care regardless of economic standing and strives to provide quality care that is equal, non-biased, personalized, and catered to you as an individual.
When not in the office I am spending time with family and friends, snuggling cats, playing in the water somewhere, or creating music. I am originally from Southern California (don’t hold it against me) and have been in love with Oregon for ten years now. I am honored to have the privilege of opening a Primary Care clinic in your community.

Eden Sisk, NCMA
Clinical Operation Manager
Born and raised right here in beautiful Oregon, I bring over six years of hands-on experience in both family medicine and urgent care as a Certified Medical Assistant. I currently serve as the Clinical Operations Manager at Perigon Medical. My goal is to ensure every patient feels welcome, cared for, and confident in the services we provide; while overseeing daily clinical operations to ensure everything runs smoothly.
When I’m not at the clinic, you’ll usually find me spending quality time with my family or binge-watching the latest Netflix series with a good cup of coffee in hand.

Roxanne Cole
Clinical Coordinator
Roxanne Cole joins Perigon Medical Clinic as our Clinical Coordinator, bringing over 10 years of experience as a medical receptionist. With a strong background in patient services and front-office operations, Roxanne serves as the first point of contact for our patients, ensuring a welcoming and seamless experience from the moment they connect with our clinic.
As a dedicated patient liaison, she manages scheduling, insurance verifications, payments, and all non-medical patient needs with professionalism and efficiency. Roxanne is committed to providing compassionate support and clear communication, helping patients navigate their care with confidence and ease.
Tea not coffee, Roxanne is our Duck faithful, and hails from “O” Nation! She loves spending time with family and friends, snuggling her “grand puppies,” bingeing true crime and of course, watching football.
